The South Riverdale Community Health Centre offers support and information for early detection and living with cancer.
Information, programs and services to help you manage your diabetes, feel better and avoid complications. All programs and services are free, and open to caregivers and family.
Treatment, group support and peer education for hepatitis C (hep C) patients. Issues include: treatment options diet and lifestyle harm reduction and preventing transfer mental health and depression
A specialized healthcare provider works with the health services and urban health teams to provide one-to-one care and education on asthma. The Primary Care Asthma Program (PCAP) is part of Ontario’s Asthma Plan of Action.
